Access to Care
The service utilises a team of skilled clinicians, including nurses, paramedics, and occupational therapists, to guide patients through the healthcare system and coordinate their care.
They facilitate prompt discharge, help prevent hospital admissions, and connect patients with community health teams.
The integrated care hub is staffed from 8 a.m. to 9 p.m. on weekdays and from 8 a.m. to 6 p.m. on weekends and holidays, with Medvivo Out of Hours staff providing support outside of these hours.
Admission Avoidance
The single point of access is crucial in identifying patient treatment pathways, preventing hospital admissions, and acting as an interface between health and social care providers.
Although the term ’admission avoidance’ is often used it is important to remember all of us have a duty to try and make sure that wherever possible, patients are treated within their own homes or as near to them as practicable.
Access to Care Case Management
The single point of access is a referral and coordination mechanism that collaborates with community health, hospitals, voluntary, and social care services to ensure patients receive appropriate care at the right time.
