Medvivo hosts the BSW Connect High Intensity User Service in Bath, North East Somerset, and Swindon, collaborating with Wiltshire Centre for Independent Living (Wiltshire CIL), to provide support and guidance for frequent healthcare use.
Guidance and support
The BSW Connect High Intensity User Service Service in Bath and North East Somerset, Swindon and Wiltshire, was commissioned to Medvivo in 2024. The service works together with Wiltshire CIL to support and guide people who may use healthcare services more often than usual.
NHS services identify the people in the area who have visited the emergency department most frequently. The team uses the personalised care model and supports high intensity users with the aim of improving their experience and outcomes and reducing ED visits and other areas of high impact.

This model aims to give people more choice and control over their health and care.
